2019 Medical Information

Medical Storage Form: If your child needs to store and Epi-pen or inhaler, review the following instructions:

1. Click on the link below and bring a signed copy to registration the first morning of camp. Be sure to include instructions for emergency administration.                                              Medication Administration Form
2. Put the Epi-pen or inhaler into a Ziploc bag and label it with your child’s first and last name, his camp, your first and last name, and your phone number.  It will be stored at the camp table.  For a camper to keep and self-administer, fill out the section on the form.
3. On the last day of camp, please pick up the Epi-pen or inhaler  and sign the return form. Your child cannot pick it up by himself.
4. IMPORTANT: We cannot not store or administer any over-the-counter or prescribed medications.
